Mary Jo McClain, with HCI Hospice Care Services in Knoxville, tells KNIA KRLS News that making those healthcare decisions is not complicated, nor is it time-consuming. She says it’s basically two pieces of paper, that do not require an attorney to complete. She says the form can be found on the Iowa State Bar Association website, or HCI Care Services will give you a packet with the form and instructions, for free, if you want to pick one up there. She says the form does need to be notarized, or be signed by two witnesses that are not going to be assigned to be your care decision makers.
